By signing this form, I appoint Do Well Limited t/a Tax Link as my registered tax agent. Authority is given to Bruce McDowell, Do Well Ltd and staff to:

1. Act on my behalf with Inland Revenue in respect of all tax types (except child support), and obtain information from Inland Revenue about those tax types through all media and communications channels.
2. Sign and file tax returns and other documents with Inland Revenue on my behalf where this is needed for the work I have engaged Tax Link to do.
3. Obtain any information from financial institutions and any other party that Tax Link may determine is necessary to complete my financial statements and/or tax returns, pursuant to the Privacy Act 2020.

I consent to any personal information I provide being used by Tax Link for the introduction of information and other services to me.

Tax Link's fees are determined by reference to our schedule of minimum charges, employee daily time sheets and disbursements incurred. Tax Link's time-based fees are $150 per hour plus GST if any, plus an agency fee of $75 plus GST if any for each IR3 Tax Return.

All Tax Link fees are due for payment within 14 days from the date on which we first notify you of any amount due. Income tax returns may be lodged upon payment of our account, which is taken as your acceptance of the accuracy of the return. Overdue accounts will be charged interest at 2% per month, and Tax Link may recover any costs it incurs should an account become delinquent or fall outside these terms.
